Rule 9 of the Conservation Review Board’s Rules of Practice and Procedure state that any individual appearing before the Board as a party’s representative must complete a Form 1-Representative of a Party-Commencement of Authorization form. Forms for Municipalities
The Ontario Heritage Act requires the council of a municipality to refer certain disputes to the Board for a hearing and report. The following Checklists reference the specific documents that a referring party must submit to the Board for a proceeding to commence. For assistance, please contact the Board. To be used when a Party wishes to request that the Review Board issue a summons to an individual to appear before the Review Board at an oral Hearing. This form can be submitted electronically, by facsimile or by regular mail as an attachment.
To be used when a Party wishes to request that the Review Board issue a summons to an individual to participate in a Hearing before the Review Board to be held by telephone conference call. This form can be submitted electronically, by facsimile or by regular mail as an attachment.
If the witness does not obey the summons, a judge may issue an arrest warrant.
On April 1, 2015, the Review Board adopted Rules of Practice and Procedure which include the obligation that any witness statement required by the Review Board to be served on other Parties and filed with the Review Board, must include a signed form in accordance with Form 6 in Appendix A of the Rules (Acknowledgement of Expert’s Duty), where the witness is to give opinion evidence. This form can be submitted electronically (by forwarding it as an email attachment in either Adobe PDF or Word format, by facsimile or by regular mail.
